The Professor Guido Silvestri on Covid-19: “A hopeless virus”

Senigallia 05/04/2020The Prof. Guido Silvestri is from Senigallia and he is teacher at Emory University in Atlanta and one of the founders of the Pact for Science. He is among the doctors involved in the front line in the United States for the Covid-19 study. Here is the point on the state of knowledge of the terrible virus that is sowing thousands of victims around the world.

“I write a short post because it is a Friday night that comes to the end of a week of battle. I do it to share a reflection on how our “nightmare” SARS-CoV-2, which in these weeks is having its quarter of hour of notoriety, it actually has NO HOPE against our science. I was thinking that, unlike HIV – an enormously more insidious enemy who in thirty years has made about 35 million deaths and for which we still have neither a definitive cure nor a vaccine – SARS-CoV-2 is a virus incapable of hiding (in technical terms: of integrating itself into the host genome) and not very good at mutating, therefore remaining much more vulnerable to the host’s immune response. So, if it is unfortunately inevitable that COVID-19 will still cause many deaths in the coming weeks and maybe months, it is even clearer that it will soon be DEFEATED by our ability to study and neutralize it.

Living immersed full time in this frantic fight against the new virus – and being in constant contact with the greatest experts in the world in this field – I could really write pages and pages on the progress we make every day. There is little talk about the progress in the media and often in a confused way – while the headlines of newspapers and TV are wasted on bad news. Since it is late and I am tired, I will say only a few of the things that are being done, in summary, and a little in bulk. With each passing day our knowledge of the virus, of the disease and of the pathogenic mechanisms (as well as of the protective ones) increases. Virological and serological tests to determine the presence of the virus and the state of antibody immunity become increasingly specific and sensitive.

New antiviral drugs are constantly tested (attention to EIDD-2801) and at the same time drugs that block the hyper-inflammatory response which is the basis of the most serious lung complications (attention to baricitinib). Neutralizing monoclonal antibodies are developed for therapeutic use while studying the healing effect of the plasma of “cured” patients. Several vaccine candidates are tested on animal models and will soon go to the clinical stage. All while our friends resuscitators, infectious disease specialists, pulmonologists etc. they get better at handling the severe complications of COVID-19.

I repeat it like a broken record, but – believe me – it’s the pure truth. The presence of SCIENCE is the real, big difference between today and 1348 of the black death, or 1630 of the Manzonian plague, or 1918 of the Spanish influence. The presence of SCIENCE is the fundamental reason why SARS-CoV-2 is a hopeless virus.

Guido Silvestri, senigalliese e docente alla Emory University di Atlanta e tra i fondatori del Patto per la Scienza, è tra i medici impegnanti in prima linea negli Stati Uniti per lo studio del Covid-19. Ecco il punto sullo stato di conoscenza del tremendo virus che sta seminando migliaia di vittime in tutto il mondo.

“Scrivo un post breve perché è un venerdì notte che arriva alla fine di una settimana di battaglia. Lo faccio per condividere una riflessione su come il nostro “incubo” SARS-CoV-2, che in queste settimane sta avendo il suo quarto d’ora di notorietà, non abbia in realtà NESSUNA SPERANZA contro la nostra scienza. Riflettevo sul fatto che, a differenza di HIV – un nemico enormemente più insidioso che in trent’anni ha fatto circa 35 milioni di morti e per il quale non abbiamo ancora né una cura definitiva né un vaccino – SARS-CoV-2 è un virus incapace di nascondersi (in termini tecnici: di integrarsi nel genoma dell’ospite) e non molto bravo a mutare, quindi rimanendo molto più vulnerabile alla risposta immune dell’ospite. Per cui, se è purtroppo inevitabile che COVID-19 farà ancora molti morti nelle prossime settimane e forse mesi, è ancora più chiaro che sarà presto SCONFITTO dalla nostra capacità di studiarlo e neutralizzarlo.

Vivendo immerso a tempo pieno in questa lotta frenetica contro il nuovo virus – ed essendo in costante contatto con i più grandi esperti al mondo in questo campo – potrei veramente scrivere pagine e pagine sui progressi che facciamo quotidianamente. Progressi di cui sui media si parla abbastanza poco e spesso in modo confuso – mentre si sprecano i titoloni di giornali e TV sulle brutte notizie. Siccome è tardi e sono stanco, dirò solo alcune delle cose che si stanno facendo, in sintesi, e un po’ alla rinfusa. Ogni giorno che passa aumenta la nostra conoscenza del virus, della malattia e dei meccanismi patogenetici (come anche di quelli protettivi). I test virologici e sierologici per determinare la presenza del virus e lo stato dell’immunità anticorpale diventano sempre più specifici e sensibili.

Si sperimentano di continuo nuovi farmaci antivirali (attenzione a EIDD-2801) ed al contempo farmaci che bloccano la risposta iper-infiammatoria che è alla base delle più gravi complicanze polmonari (attenzione a baricitinib). Si sviluppano anticorpi monoclonali neutralizzanti per uso terapeutico mentre si studia l’effetto curativo del plasma dei pazienti “guariti”. Diversi candidati vaccini sono testati sui modelli animali e andranno presto in fase clinica. Il tutto mentre i nostri amici rianimatori, infettivologi, pneumologi ecc. diventano sempre più bravi a gestire le complicanze severe di COVID-19.

Lo ripeto come un disco rotto, ma – credetemi – è la pura verità. La presenza della SCIENZA è la vera, grande differenza tra oggi ed il 1348 della morte nera, o il 1630 della peste manzoniana, o il 1918 della influenza Spagnola. La presenza della SCIENZA è il motivo fondamentale per cui SARS-CoV-2 è un virus senza speranza.
